The Main purpose for the form is to get Fidelity: 

1. Your info;

2. Amount giving (and type of asset);

3. Signature.

This is how Fidelity will get you a tax statement at the end of the year, and it needs to accompany your first donation. I have pre-filled some of the blanks (account number and asset allocation). You need to complete Section 1, and any other sections depending on how you are going to give. You should only need to fill this form out once, though you might want to each time if you give complicated donations (stock grants, etc). You can mail it in, or email it to 8776654274@fidelityfaxmail.com. Copy me on any emails so I can help facilitate.  Also, you’ll notice that there are a few options for assets that you can give. The most simple is cash (or cash equivalents/checks, etc). At this time, there are two ways to give cash. Either (1) send a paper check, or (2) set up an electronic funds transfer from your bank to Fidelity. Fidelity is unable to initiate that process for you, and as yet cannot accept credit card donations (this might be available down the road). 

 

If you are giving stock or some other asset, I would advise sending the contribution form with each contribution for the avoidance of doubt. They really push donating appreciated stock, so that could be something for us to lean more towards going forward. Some members have done it recently and it has great tax benefits. 

 

regardless of how you give what you give, you should reference "Giving Account Number 1095172” on your donation. This number should be used on all donations (I have pre-populated it on the attached form).
